In the film The Shining (1980), directed by Stanley Kubrick, the character played by Jack Nicholson ends up not so unlike Jack Nicholson himself- kinda ugly and manic-looking.

At the end of The Empire Strikes Back, it is revealed... that George Lucas is a prevert.  Also very bad at coming up with names.  Hell, though, that's revealed as early as Star Wars.  Also, Tom is right about those Rebel Pilots.  Not drawn from the great character actors, as you might expect.
